Consent Forms
Retaining Consent Forms
Copies of consent forms will be retained for one month.
After this time, consent forms will be scanned and shredded.
Scanned copies will be retained for 7 years, in compliance with SPG 604.1.
Investigator Responsibility for Consent Forms
In compliance with Human Subject guidelines, it is the responsibility of the investigator to retain original copies of their subjects' consent forms.
